Use the 'Site Builder' when viewing one of your images in Element, and click on the blue Edit button above the image. Set the 'Image Info' drop-down menu to ON in the left hand panel. This will display the Headline in bold, and the Caption/Description beneath it in regular text. In the case of Nick Hawkins, there appears to be no Headline, so just the Caption/Description is showing.

Is there a way I can just display the caption and not the headline? The file name seems to be coming up as the headline despite the boxes being left empty for headline and title in the IPTC Metadata panel.

OK, try using a non-breaking space for your headline (on a Mac that's Alt+Space bar). This will force it to display a blank headline on the image view.
